Medical Warranty Questionnaire
If you are unable to comply with the Medical warranty questions
below, it may render any insurance issued null and void - any claims
arising may be denied Fairsure Travel Insurance operates on the
basis that you are healthy, fit to travel and to undertake the trip.
Unless you declare otherwise to us, you will be held to warrant
that this is true both at the time of booking and at the time you
commence the trip.
In answering the questions, you must advise of anything that might
influence insurers in the acceptance of insurance, including the
health of a relative. If you are in ANY doubt, it should
be disclosed.You must disclose ANY reason that you are aware
of why the holiday/trip may be cancelled.
QUESTION 1 Are you on a waiting list for, receiving or have
you received hospital in-patient or out-patient treatment within
the 12 months prior to booking the trip?

2 Have you received any medical treatment (including investigation
or medication) within the past three years for:
Heart disease, a heart or circulatory related condition eg:hypertension,
angina, stroke etc.
A lung or breathing related condition other than well controlled,
or mild asthma suffered in isolation. Diabetes (other than well
controlled with no treatment/ hospitalisation in past year).
Have you been diagnosed with any anxiety state or depression (within
the past 12 months only)
If you have answered Yes to either of the above then YOU
or your Travel Agent must contact Accident & General on (O1)
874 8458 for approval.
Your answers to the above questions will form part of the policy
and will be incorporated into it.
PLEASE NOTE
The Underwriters reserve the right to decline cover related to any
existing condition and/or offer limited cover only.
There is NO cover for a claim arising due to a condition
which has been diagnosed as terminal. There is NO cover if
you are travelling against the advice of a doctor.
There is NO cover if you are travelling for the purpose of
obtaining medical treatment. Cover applies only after
your policy has been issued by your Travel Agent.
