Single Trip Fairsure TravelSafe option notes
1.
Children 2 - 17: 50% of Adult premium / Can be booked without adult
2. Adult 18 - 65: Standard adult premium
3. Adult 65 - 69: 2 times adult premium (Max duration 185 days)
4. Adult 70- 74: 2.5 times adult premium (Max duration 185 days)

5. Adult 75+: 3 times adult premium (Max duration 185 days)
6.
Adult 80+: 3.5 times adult premium (greater than 30 days requires authorisation)
7.
Family Policy: 2.5 times adults premium(2 adults and all children up to and including 23 years of age in full time education)
8. Infant up to 2 years of age: FREE of charge(with premium paying adult)

Maximum length of stay per trip: 30 days

DECLARATION

1. Prior to and at the time of buying the insurance policy:
(a)You are not aware of any reason why the holiday/trip should be cancelled or curtailed.
(b)You are not receiving or awaiting treatment as a hospital in-patient
(c)You are not travelling against the advice of a Medical Practitioner or for the purpose of obtaining medical treatment or if a terminal prognosis has been given.
(d)You have obtained a medical opinion on the advisability of taking the holiday/trip where you have received medical treatment as a hospital in-patient during the 6 months preceding the holiday/trip booking.
2. Where you have or are receiving medical treatment, investigation or medication for a Cardio-vascular, Cardio-respiratory, Cerebro-vascular and/or a malignant heart disease that occurred during the 3 years prior to the booking of the holiday/trip, a medical declaration must be completed by your Medical Practitioner confirming your ability to travel and endure the holiday/trip.
3.
You must notify the issuing agent of any such reason above arising between the date of issue of this Policy and the start of the holiday/trip.

NOTE: A material fact is a fact likely to influence us in the acceptance, assessment or continuance of this Insurance (for example, your own state of health or that of an immediate relative). If there is any doubt as to whether a fact is 'material' then it should be disclosed. Please contact us for clarification.
